Merton’s Hospital Discharge Team receives positive feedback from survey

Merton Council’s Hospital Discharge Team (HDT) received extremely positive feedback about their service in the evaluation of their Customer Satisfaction Survey for the year 2000.

The hospital discharge team provides short-term care and rehabilitation in patients’ homes following their discharge from hospital for up to six weeks. The team also work extensively with terminally ill clients who they support with continuous intensive care packages.

The survey results showed of the 166 patients who responded 165 felt the service had been sufficiently flexible to meet their needs and 159 patients felt they received support from the service as quickly as they needed it upon discharge.

Councillor Peter McCabe, Cabinet Member for Care Services said: “I am delighted with the positive results of the survey. It confirms that the Hospital Discharge team offer a valuable and successful service to the community. I am confident they will continue to be successful.”

Patients are visited and assessed either prior to or on the day of discharge from hospital and are then reviewed on a regular basis for up to six weeks. If longer-term care is required patients will be referred to the appropriate teams.
